Connected Care (CC3) is a program that was born out of necessity when we realized our clients needed considerable assistance with daily living. The program consists of three divisions creating a comprehensive trifecta of services: skilled nursing, caregiving, and transportation.

Our non-medical transportation programs help clients get to their doctors appointments, visit the grocery store, or go wherever they need to be!

From skilled nursing and home health aids to physical therapy, our experienced professionals are licensed in their fields and will build a care plan specially for you, based on your personal challenges and goals.

CC3 offers in-home help to help seniors and adults with disabilities remain living in the comfort of their own home.

Home Health Care is designed to give consistent support to individuals who are unable to care for themselves adequately and maintain daily activities. A better quality of life for our clients is the goal. CC3 provides their clients the opportunity to hire people they trust such as friends or a family member to care for them or we carefully find a perfect caregiver match. A case manager is assigned to each individual clients cast to help guide you through the process assuring continuity of care.

100% Medicare & Medicaid Covered
For eligible Medicare beneficiaries, there are no out-of-pocket costs for home health services. A wide range of other insurances are also accepted.
